1. Introduction
A Las Vegas Medical Group ("A Las Vegas Medical Group," "ALVMG," "we," "us," or "our") provides adult medicine, family practice, and pediatric health care services in the Las Vegas and Henderson, Nevada area. We respect your privacy and are committed to protecting the personal information you share with us.
This Privacy Policy explains what information we collect through our website at https://alasvegasmedicalgroup.com (the "Site"), through our text message (SMS) program, and through other electronic communications with us; how we use that information; who we share it with; and the choices available to you.
Important: This Policy Is Not Our HIPAA Notice of Privacy Practices
A Las Vegas Medical Group is a "covered entity" under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations ("HIPAA"). Your protected health information ("PHI") — the medical and health information we create, receive, and maintain in connection with your treatment, payment for that treatment, and our health care operations — is governed by our separate Notice of Privacy Practices, not by this Privacy Policy.
Our Notice of Privacy Practices describes your rights regarding your medical records and how we may use and disclose PHI. It is available at our office, upon request by calling (702) 733-0744.
Where this Privacy Policy and our Notice of Privacy Practices conflict as to protected health information, the Notice of Privacy Practices controls.
Please also note that our patient portal is hosted by athenahealth, a third-party service provider. Your use of that portal is additionally governed by athenahealth's own privacy policy and terms of use.
2. What Personal Information We Collect
2.1 Information You Provide Directly
We collect information you voluntarily give us, which may include:
- Identifiers and contact information: your first and last name, mailing address, email address, home phone number, and mobile phone number.
- Demographic information: date of birth, gender, and preferred language.
- Appointment and scheduling information: the provider or practice (Adult Medicine or Pediatrics) you wish to see, requested appointment dates and times, and the general reason for your visit if you choose to provide it.
- Insurance and billing information: health plan name, member and group identification numbers, guarantor information, and payment or billing details you submit to us.
- Health-related information you choose to submit: information you include in a contact form, patient form, appointment request, secure message, or text message to us.
- Parent or guardian information: for our pediatric patients, the name and contact information of the parent, legal guardian, or authorized representative.
- Communications: the content of emails, contact form submissions, voicemails, and text messages you send to us, and our responses.
2.2 Information Collected Automatically
When you visit our Site, certain information may be collected automatically by us or by our service providers, including:
- Internet Protocol (IP) address and approximate general location derived from it
- Browser type and version, operating system, and device type
- Pages viewed, links clicked, time spent on pages, and the date and time of your visit
- The referring website or search term that brought you to our Site
- Cookie identifiers and similar technologies (see Section 7)
2.3 Information From Other Sources
We may receive information about you from other sources, including referring physicians and other health care providers, your health plan or insurer, clearinghouses and other payers, laboratories and imaging facilities, your parent or legal guardian, and your authorized personal representatives. Information received in these ways is generally protected health information governed by our Notice of Privacy Practices.
2.4 Sensitive Information
Because of the nature of our services, information you provide may be sensitive, including health, medical, or insurance information. Please do not send detailed medical information, test results, or other sensitive health information through our website contact forms, standard email, or text message. These channels are not encrypted and are not secure. Use our patient portal or call our office at (702) 733-0744 instead.
3. How We Use Personal Information
We use the personal information we collect for the following purposes:
- To provide health care services — to schedule, confirm, reschedule, and cancel appointments; to register you as a patient; to coordinate your care; and to respond to your clinical questions through appropriate secure channels.
- To communicate with you — to respond to your inquiries, contact form submissions, calls, emails, and text messages; to send appointment reminders and confirmations; to deliver practice notifications such as office closures, schedule changes, and new provider or service announcements; and to send follow-up and recall reminders such as annual physicals, well-child visits, and immunization due dates.
- For billing, payment, and insurance — to verify eligibility and benefits, submit claims, obtain payment, collect outstanding balances, and answer billing questions.
- For health care operations — quality assessment and improvement, care coordination, staff training and evaluation, business planning, and general practice administration.
- To operate, maintain, and improve our Site — to understand how visitors use the Site, diagnose technical problems, improve content and functionality, and protect the security and integrity of our systems.
- To protect rights and safety — to detect, investigate, and prevent fraud, abuse, security incidents, or activity that is illegal or that threatens the safety of any person.
- To comply with law — to meet our obligations under HIPAA, Nevada law, and other applicable federal, state, and local laws, and to respond to lawful requests from courts, regulators, and law enforcement.
We do not sell your personal information, and we do not rent or trade your personal information. We do not sell, rent, or share your personal information with third parties for their own independent marketing purposes.
4. Who We Share Personal Information With
We share personal information only as described below.
4.1 Service Providers and Business Associates
We share information with vendors that perform services on our behalf, including:
- Electronic health record, patient portal, and practice management providers (including athenahealth)
- Appointment reminder, text messaging (SMS), and communications platforms
- Website hosting, maintenance, and analytics providers
- Billing, coding, claims processing, and collection services
- Information technology, data storage, and security providers
- Professional advisors, including attorneys and accountants
Where these vendors handle protected health information, they are our "business associates" under HIPAA and are bound by written agreements requiring them to safeguard that information and to use it only for the purposes we specify.
4.2 Health Care Providers, Health Plans, and Payers
We share information with other physicians, specialists, hospitals, laboratories, pharmacies, imaging centers, and other providers involved in your care, and with your health plan, insurer, or other payer for eligibility verification, claims, payment, and related purposes.
4.3 Parents, Guardians, and Authorized Representatives
For minor patients, we share information with parents and legal guardians as permitted or required by Nevada and federal law. We also share information with individuals you have authorized in writing to receive it.
4.4 Legal and Safety Disclosures
We may disclose information when required or permitted by law, including in response to subpoenas, court orders, or other lawful process; to public health authorities; to health oversight agencies; in connection with reports of abuse, neglect, or domestic violence; and to avert a serious and imminent threat to health or safety.
4.5 Business Transfers
If our practice is involved in a merger, acquisition, reorganization, sale of assets, or similar transaction, patient and other information may be transferred as part of that transaction, subject to applicable law and continued protection of your information.
4.6 With Your Authorization
We share information in any other manner only with your written authorization, which you may revoke at any time as described in our Notice of Privacy Practices.
4.7 SMS Consent Is Never Shared
SMS consent is not shared with third parties. Your consent to receive text messages from A Las Vegas Medical Group, and the mobile phone number you provide for that purpose, are not shared, sold, rented, or otherwise disclosed to third parties or affiliates for their marketing or promotional purposes. Mobile information is shared only with the service providers that help us deliver the messages you have asked to receive, and those providers are contractually prohibited from using it for any other purpose. No mobile information will be sold or shared with third parties or affiliates for marketing or promotional purposes.
5. Text Messaging (SMS) Privacy
If you provide your mobile telephone number and opt in to our text message program, we use that number to send you the categories of messages described in our SMS Terms & Conditions — such as appointment reminders and confirmations, practice and account notifications, care follow-up and recall reminders, and replies to messages you send us.
- Participation is voluntary. You are never required to consent to receive text messages as a condition of receiving treatment from us, of purchasing any good or service, or of using this Site.
- Message frequency varies. Message and data rates may apply.
- You may opt out at any time by replying STOP to any message from us. You may also call our office at (702) 733-0744 to be removed.

- SMS consent is not shared with third parties. See Section 4.7 above.
Please keep in mind that standard SMS is not a secure or encrypted channel. Do not send us detailed medical information by text. If you change or give up your mobile number, please tell us promptly so that messages intended for you are not sent to someone else.

6. How We Protect Information
We maintain administrative, physical, and technical safeguards designed to protect personal information against unauthorized access, use, alteration, disclosure, and destruction, consistent with the HIPAA Security Rule and applicable Nevada law. These safeguards include access controls and role-based permissions, encryption of protected health information where required, workforce privacy and security training, secure disposal practices, and business associate agreements with vendors that handle protected health information.
No method of transmission over the Internet or method of electronic storage is completely secure, however, and we cannot guarantee absolute security. Email, website contact forms, and text messages are not secure channels — please do not use them to send sensitive health information.
7. Cookies, Analytics, and Tracking Technologies
Our Site may use cookies, pixels, tags, and similar technologies to operate the Site, remember your preferences, and understand how visitors use our pages. We use these technologies for essential Site functionality and for basic, aggregated traffic analytics.
Most web browsers allow you to refuse or delete cookies through your browser settings. If you disable cookies, some portions of the Site may not function properly.
We do not use tracking technologies on our Site to disclose information about your visits to third parties for advertising purposes.
Do Not Track
Some browsers transmit "Do Not Track" signals. There is currently no uniform industry standard for interpreting these signals, and our Site does not respond to them at this time.
8. Third-Party Websites and Services
Our Site contains links to third-party websites and services that we do not control, including our patient portal hosted by athenahealth, our Facebook page, our Yelp listing, and educational and health resource sites we link to from our Resources and Patient Forms pages. This Privacy Policy does not apply to those third parties. We encourage you to review the privacy policy of any site you visit through a link from ours.
9. Data Retention
We retain personal information for as long as necessary to fulfill the purposes described in this Privacy Policy and to meet our legal, regulatory, accounting, and reporting obligations. Medical records are retained in accordance with Nevada law and applicable federal requirements. Website and analytics data are generally retained for a shorter period. When information is no longer needed, we dispose of it securely.
10. Children's Privacy
We provide pediatric care, and we collect information about minor patients from their parents, legal guardians, and authorized representatives in the course of providing treatment. That information is protected health information governed by our Notice of Privacy Practices and applicable law.
Our Site is not directed to children under 13 for the purpose of collecting personal information online, and we do not knowingly collect personal information online directly from children under 13 without parental consent. If you believe a child has provided personal information through our Site without such consent, please contact us at (702) 733-0744 and we will take appropriate steps to delete it.
11. Your Rights and Choices
11.1 Rights Regarding Your Medical Records
Your rights concerning your protected health information — including the right to inspect and obtain a copy of your records, request an amendment, request an accounting of certain disclosures, request restrictions, request confidential communications, and file a complaint — are described in our Notice of Privacy Practices.
11.2 Communication Preferences
- Text messages: reply STOP to any message from us, or call (702) 733-0744.
- Marketing or informational emails: use the unsubscribe link in the message, or call us. Note that we may still send you non-promotional messages about your care, appointments, and billing.
- Phone calls: tell our staff which number and method you prefer, and we will accommodate reasonable requests for confidential communications.
11.3 Access, Correction, and Deletion
You may request access to, correction of, or deletion of personal information we hold about you by contacting us using the details in Section 14. We will respond as required by applicable law. Note that we may be legally required to retain certain records — particularly medical and billing records — even after a deletion request.
11.4 Nevada Residents
Nevada law (NRS Chapter 603A) gives Nevada consumers the right to direct certain website operators not to sell covered information collected about them. We do not sell your covered information, and we have no plans to do so. If you nonetheless wish to submit a verified opt-out request, email us at info@alasvegasmedicalgroup.com with the subject line "Nevada Opt-Out Request."
Nevada's consumer health data privacy law (SB 370, codified at NRS Chapter 603A) contains exemptions for protected health information held by HIPAA covered entities. Health information we maintain about you as a patient is governed by HIPAA and our Notice of Privacy Practices.
11.5 Residents of Other States
If you reside in a state with a comprehensive consumer privacy law, you may have additional rights, such as the right to know, correct, delete, or obtain a copy of personal information, and the right to opt out of sale or targeted advertising. Many of these laws exempt protected health information held by HIPAA covered entities. To make a request, contact us using the information in Section 14. We will not discriminate against you for exercising any privacy right.
12. Individuals Outside the United States
Our Site and services are intended for individuals located in the United States. Information we collect is stored and processed in the United States, which may have different data protection rules than your country of residence.
13. Changes to This Privacy Policy
We may update this Privacy Policy from time to time. When we do, we will revise the "Last Updated" date at the top of this page and post the updated policy on this Site. Material changes will be identified more prominently where appropriate. Your continued use of the Site after an update means you accept the revised policy.
